These are the steps you need to be sure to follow if your application is for the Minnesota Medical Cannabis Program.

Before you are able to apply to the Missouri marijuanas card program, a doctor, specialist assistant or advanced registered nursing must first certify that your health condition is legitimate.


  • To make an appointment, contact your doctor, specialist assistant, or advanced practice registered nurse to arrange for an appointment. These are the only medical professionals that can confirm your condition.

  • The OMC does not keep track of those who are participating in the program. You are responsible for scheduling appointments with registered health professionals.

  • The Acknowledgement of Patient Email and Patient Form is available for download and printing (PDF).

  • It is necessary to fill out the following areas on the Patient Email and Acknowledgement Form.

  • Please enter your email address if you would like to receive messages from a private source regarding your health. If you don't have an email account at work, consider creating one before your appointment.

  • This form should be taken to your appointment, and then given to your doctor.
